What Are Cataracts?

A cataract is a clouding of your natural lens inside the front portion of your eye. The lens sits just behind the colored part of the eye called the iris. The lens is made up of many proteins that change and lose transparency, and this is known as cataract formation. It can form for many reasons, including age, medical conditions, steroid use, trauma, etc. Though cataracts are thought of as a condition for the older population, many are surprised to learn that over 20% of those who have cataract surgery are under 65 years of age.

What is Cataract Surgery?

Cataract surgery becomes necessary when cataracts begin to significantly affect your quality of life. It is a straightforward procedure in most cases, where your cataract surgeon removes the natural lens and replaces it with an artificial IOL. After creating a small opening in the front surface of the eye, your surgeon will break up the natural lens, remove the pieces, then insert an artificial lens in its place. This new, clear lens allows light to pass through and focus properly on the retina. The IOL becomes a permanent part of your eye. The tiny opening in the surface of your eye will heal naturally without stitches, and most people notice an improvement in their vision right away.

Dr. John Josephson

Dr. Josephson is a Board-Certified, Fellowship-Trained ophthalmologist with a passion for ingenuity and advancement. He completed his undergraduate degree in cell biology and neuroscience at Rutgers University with High Honors and was one of the few Henry Rutgers Research Scholars. Prior to coming to Northern Virginia, he completed an Advanced Cornea, Cataract and Refractive Fellowship at the University of California San Diego. His medical training was completed at Robert Wood Johnson Medical School, New Brunswick, NJ, where he earned a Distinction in Research. He is an Associate Professor of Ophthalmology with the Georgetown University Residency program where he devotes time quarterly to train beginner surgeons the subtleties needed to master surgical technique.

Dr. Meghan G. Moroux

Dr. Meghan G. Moroux is a board-certified ophthalmologist with a focus in comprehensive ophthalmology and cataract surgery. She received her Bachelor of Science in Biology from Millsaps College, and her medical degree from the University of Mississippi School of Medicine, where she excelled academically. Dr. Moroux completed her residency training at MedStar Georgetown University Hospital/Washington Hospital Center in Washington, D.C. Dr. Moroux specializes in diabetic eye evaluations and treatment, medical and laser treatment of glaucoma, and advanced technology cataract surgery including femtosecond laser assisted cataract surgery. She believes in treating each patient as if he or she were a member of her own family and actively engages each patient in his or her own treatment plan.

Dr. Hubert H. Pham

Hubert H. Pham, MD, MS is a board-certified ophthalmologist with a focus on comprehensive ophthalmology, advanced cataract, and medical retina. Dr. Pham’s unique upbringing, as the son of Vietnamese immigrants who lived in Mexico and Zimbabwe, was formative to his desire to pursue a career in medicine. His mission has always been to alleviate suffering locally and internationally, having experienced it so profoundly in his youth. He attended Cornell University, and received his medical doctorate from Georgetown University School of Medicine. Following medical school, he completed his internship at INOVA Fairfax, where he was awarded the ‘Best Teacher Award’ in Internal Medicine. After training, he joined one of the largest ophthalmology groups in Seattle, where he led a successful practice and won ‘Top Doctor’ award multiple years in a row.

Dr. Sabita Ittoop

Sabita Ittoop MD is a board-certified, fellowship-trained glaucoma specialist. Dr. Ittoop uses cutting-edge technology to create customized treatments for her patients. Her skills include complex cataract surgeries, glaucoma laser treatments, minimally invasive glaucoma surgeries, trabeculectomies, and glaucoma drainage devices. After graduating from Cornell University, Dr. Ittoop obtained a master’s degree prior to completing her medical degree at UMDNJ Robert Wood Johnson Medical School. Dr. Ittoop returned to Columbia University for her intern year and completed her residency training in Ophthalmology at the University of Rochester. Throughout her career, Dr. Ittoop has been an advocate for education. Dr. Ittoop is the lead author of numerous chapters and peer-reviewed journals and her work has been published in the prestigious American Journal of Ophthalmology and Journal of Glaucoma.

Dr. Erin Ong

Dr. Erin Ong was born in Washington, DC and grew up in Montgomery County, Maryland. She is a board-certified ophthalmologist who completed residency training at the University of Maryland School of Medicine where she served as a chief resident. She received further training in Cornea, External Disease, & Refractive Surgery as a fellow at the number one ranking Bascom Palmer Eye Institute. Prior to her ophthalmology training, she completed an intern year at Mercy Medical Center and earned both her Doctor of Medicine and Masters in Genomic Medicine at the University of Miami Miller School of Medicine. She received her Bachelor of Science and Art in Neurobiology & Physiology and cum laude in Psychology as a double major at the University of Maryland. Dr. Ong has a special interest in cataract surgery, corneal transplants, pterygiums, and complex anterior segment surgery.

Advanced IOL Options

Once you have made the decision with your eye doctor to undergo cataract surgery, you will have to choose an IOL to replace your natural lens. Your eye doctor will help you decide which option may be best for you.

Multifocal/Trifocal IOL

The goal of this lens is to provide more independence from glasses by providing a range in focus from distance to near. These lenses use diffractive optics to allow for these ranges and are not accommodative lenses. They are not bifocal lenses. Similar to the lens of a very advanced camera, the optics of the lens allow for the extension of range. The optics allow a person to do near activities and distance activities without having to look in a certain direction as in a bifocal or progressive glasses.

Extended Vision IOL

Vivity IOL gives patients the option of range in vision without increasing risk of glare and haloes. Vivity uses wavefront technology to give a seamless range in vision as opposed to the diffractive rings used in multifocal technology. The range of Vivity typically extends from distance to midrange and some near (i.e., large print). The near vision (i.e., small print) that Vivity gives may not be as much as what would be obtained with a trifocal lens. For a patient wanting premium cataract surgery with more than one focus, the option of Vivity vs. Trifocal is a discussion to be had with your surgeon to choose which option would be ideal in your case.

Accommodative IOL

Accommodation is the process by which the lens changes shape to focus eyes from distance to near and vice versa. Presbyopia is the process where accommodation is slowly lost, and a person requires the aid of reading glasses to see objects up close. These lenses (i.e., Crystalens) were designed to allow the lens to shift and give distance and up-close vision without using diffractive optics as in the multifocal lens technology. Typically, a person can expect similar distance vision as in the monofocal lenses and some intermediate near vision. Small print will likely still require the aid of magnifiers. There is an accommodative option with astigmatism correction as well named Trulign. There may be more variation in results with these lenses.

Light Adjustable Lens

This groundbreaking technology redefines cataract surgery outcomes. The Light Adjustable Lens is a unique intraocular lens that allows post-surgery adjustments to fine-tune vision. Using UV light, the lens can be modified to achieve the desired refractive correction, providing personalized vision enhancement. This ensures precise results, minimizing the need for additional procedures.
